YOUR.MD’s ‘Breast Self-Check Guide’ TERMS AND CONDITIONS and PRIVACY STATEMENT
WELCOME TO YOUR.MD ‘Breast Self-Check Guide’
(‘Self-Check’ terms – April 2018)
Your.MD is a trade mark of YOUR.MD AS, incorporated and registered in Norway with company number 999260993 whose registered office is at c/o Advokatfirmaet Simonsen Vogt Wiig AS, Filipstad Brygge 1 , 0252 Oslo, Norway and it is offering the Your.MD Services (hereinafter referred to as: “Services’’) via its subsidiary Your.MD Ltd., a company incorporated and registered in the UK with company number 08727263 whose registered office is at Your.MD Ltd, 5th Floor, 43 Whitfield Street, London, W1T 4HD, UK (“Your.MD”).
Before you start the ‘Self-Check’ you will need to actively accept this Terms and conditions and Privacy statement (the “Terms”) and agree to our data processing practices by clicking the button “OKAY”. By doing so, you consent to the collection and usage of your data, so please read this Terms carefully. You can always withdraw your consent by sending us an email to: email@example.com.
Your.MD is offering the online self-check guide titled as ‘Your.MD ‘Breast Self-Check Guide’ (the “Self-Check”). To access the Self-check you will need to click on the link available on Your.MD website LINK or Your.MD Facebook profile LINK. The link to the Self-Check is provided by our third party provider TYPEFORM S.L., Carrer Bac de Roda, 163, 08018 Barcelona, an online software service company that specializes in online form building and online surveys. More info on https://www.typeform.com/ (“Typeform”). By clicking on the link you will enter the Self–Check. At the beginning you will be asked to enter some general information about you (see below Collection and use of information). Before you start the Self-Check we will provide you with some useful information about how you can reduce the risk of breast cancer. After that you will be able to choose how would you like to learn about doing the Self-Check properly, namely by watching a video from third party provider DiscoverySA (for more information visit https://www.discovery.co.za/portal/), with Cartoons and infographics from third party provider getnaturopathic.com (for more information visit https://getnaturopathic.com/ ) or by a Step by Step text guide from third party provider Breastcancer.org (for more information visit http://www.breastcancer.org/). At the end of the Self-Check you will be asked whether you would be interested to receive regular reminders to do your Self-Check and provide your email address for this purpose. As the last step you will have the option to rate the Self-Check experience and give additional feedback.
THE PURPOSE OF THE SELF-CHECK
The purpose of the Self-Check is to give you the opportunity to take a guided Breast self-examination that will teach you how to safely check your breasts, and help you spot the signs for when something might be wrong.
USE OF THE SELF-CHECK
BEWARE THAT YOUR.MD PROVIDES USEFUL HEALTH INFORMATION AND IT IS NOT A SUBSTITUTE FOR PROFESSIONAL MEDICAL ADVICE, DIAGNOSIS, THERAPY OR TREATMENT. BEFORE ACTING ON ANY INFORMATION YOUR.MD PROVIDES TO YOU, CONSULT WITH YOUR DOCTOR TO MAKE SURE THAT IT IS RIGHT FOR YOU. IF YOU THINK YOU MAY HAVE A MEDICAL EMERGENCY, CALL YOUR DOCTOR OR THE EMERGENCY SERVICE IMMEDIATELY.
YOUR.MD IS NOT LIABLE FOR ANY DECISION YOU MAKE SOLEY ON THE INFORMATION PROVIDED BY YOUR.MD OR THE LACK OF IT. YOUR.MD DOES NOT PROVIDE MEDICAL DIAGNOSIS OR TREATMENT, BUT RATHER HEALTH INFORMATION THAT ENABLES YOU TO DECIDE ON APPROPRIATE FURTHER ACTIONS OR OMISSIONS, WHICH MUST BE PRESCRIBED BY PROFESSIONAL MEDICAL SERVICES.

As a convenience to you, our Self–Check contains links form websites operated by persons or entities other than Your.MD. A link form Your.MD to another website does not imply or mean that Your.MD endorses the content on that website or the operators of the website. Your.MD does not review and specifically disclaims responsibility for materials posted or activities that occur on linked sites. You are solely responsible for determining the extent to which you use any content at any website to which you link from Self-Check, and you agree not to hold Your.MD responsible for the content or operation of such websites.
The video is propriety of Discovery SA and it is published on YouTube under Standard YouTube Licence that gives us the right to use, reproduce, distribute, prepare derivative works of, display and perform the content of the video (the video is available on https://www.youtube.com/watch?v=clOEgvgUxfk ).
The cartoon and infographic self-check is propriety of Getnaturopathic.org and is available on https://getnaturopathic.com/breast-self-exam-breast-cancer-detection/.

COLLECTION AND USE OF INFORMATION
In order for us to be able to provide Self-Check we need to collect your technical and analytical data - how you use our services (e.g. what is users most popular way of learning about self-examination, effectiveness of guides ecc.), as well as some general information about you (namely your age, general health self-rating (1-10), information about previous self-check (Y/N), your satisfaction with the Self-Check) and your personal data (namely your email address).
By answering the general questions you will help us understand what type of users are interested in our guides, what is the most popular way to learn and the effectiveness of the guides.
We will use your email address to send you regular reminders solely if you agree to it and for our internal analytics. We will not share it with any third party except with our Survey provider Typeform. You can request that we delete your email at any time.
Data storing, security and deletion
We follow generally accepted industry standards and internal procedures to protect information submitted to us, during transmission, storing and processing. We store your information for as long as needed to provide our Service. We delete the logs we keep of the IP addresses you have used after approximately 6 months.
We make sure we do not collect more information than is needed to provide the Self-Check to you and we strive to limit our providers to do the same. You can request access, deletion and correction of your data at any time by sending an email to firstname.lastname@example.org.
All data that you provide during the Survey will be collected and stored by Typeform for us. We are responsible for that data and we manage it. Typeform also collects usage data about you whenever you interact with their services and records information about the source that referred you to the Survey (i.e. a link on a website or in an email).
Typeform records the email address in order to send you a Survey invitation email. You may opt-out of receiving email invitations to complete the Survey that are sent by Your.MD via Typeform. Typeform uses your data to review, investigate, and analyse how to improve the service they provide. They may also collect and analyse your data to monitor, maintain and improve their services and features.
